Treatment for Adolescents With Depression Study (TADS) - RCT pivotal NIMH, N=439, 12-17 anos con TDM. A 12 semanas: combinacion FLX+CBT 71% respuesta, FLX solo 61%, CBT sola 43%, placebo 35%. Combinacion NNT=3 vs placebo. Tasa de ideacion suicida 14.7% en TADS, mayor en FLX-solo (15.3%) que combinacion (8.4%), apoyando rol protector del CBT cuando se anade SSRI. Trial fundacional citado en todas las guidelines posteriores (AACAP, NICE, GLAD-PC).
